CAREGIVER COVID-19 HEALTH CERTIFICATION SURVEY

Dear Caregiver,

Due to the recent spike of COVID-19 cases, the Health District has instructed us that caregivers must do a COVID-19 health survey each day before work.

Starting on Monday, July 27th, you will NOT be able to clock in with a client unless you do a health survey. The health survey will include the following three questions:

1. In the last 24 hours, have you had symptoms associated with COVID-19, such as cough, shortness of breath, or fever?
2. In the past 14 days, have you been in close contact (within 6 feet for over 15 minutes) with anyone diagnosed with COVID-19 without appropriate protection?
3. Have you been advised to personally quarantine or isolate by a health care provider, another agency or employer?

You can respond to the survey by using Telephony or the Caregiver Mobile App.

Telephony Instructions:

• Call 1-877-978-1272
• Enter Caregiver Number followed by the # sign
• Press 4 to fill out the daily health survey
• For “YES” – press “1”; for “NO” – press “2”
NOTE: If you try to clock in at client’s house without completing the survey, the telephony system will take you automatically to the Health Survey Questions
• If you’ve answered “No” to each of the questions, the system will then allow you to clock in as you usually do.

Caregiver Mobile App Instructions:

• Click on the red “ALERT: Health Survey”
• Click on “Tasks” below
• Click on each question and select answers “Yes” or “No”
• In the end, click “Save”
• If you’ve answered “No” to each of the questions, the system will then allow you to clock in as you usually do.

If you answer “Yes” to any of the questions, you will be identified as a health risk and you won’t be able to clock in and work with the client. Call your supervisor immediately!

If you’ve made a mistake but cannot reach the supervisor immediately to fix it, you may leave a message for the supervisor and start working with a client. The supervisor will be able to override the survey and clock you in manually.

NOTE:

• You don’t need to be at a client’s residence to do the heath survey. You can respond to the survey in advance by calling from your phone or using the app before you go to work.
• You need to do the survey only once per day even if you work with multiple clients.
