Sunrise is now offering a new behavior support program, ECS, to support successful community placements and aim to prevent hospital readmission. Sunrise is partnering with Home and Community Services and long term care facilities across Snohomish and Island Counties.
The Sunrise Behavior Support Team is a multi-faceted team including Licensed Clinical Social Workers, Licensed Behavioral Health Counselors, a Psychiatric Nurse Practitioner and Geriatric Behavioral Health Specialists.
The main goal of this program is to help individuals participate and stabilize in their community.
