Adult Family Homes

What They Are
Adult family homes (AFHs) are licensed residential care homes that house, feed, and provide personal care to a small number of residents. In Washington State, they can serve up to six non-related residents at a time.
Different AFHs provide different levels of care. Some provide higher levels of personal care, intermittent nursing care from a part-time nurse, or assistance with medications, while others offer more limited services. Some AFHs also specialize in caring for people with developmental disabilities, dementia, mental illness, or other conditions.
An AFH can be run by a family, but many are run by individuals or business partners with employees. Depending on the AFH, residents may be able to have pets or communicate with other residents and caregivers in a language other than English.
Who They’re For
AFHs are often a good fit for individuals who don’t need a high level of daily medical care. They create a homey setting that feels more comfortable than an assisted living facility for some people.
Cost
Adult family home pricing can vary quite a bit. According to the Adult Family Home Council, pricing in Washington state usually starts at $4,500/month for a shared room, and a private room often starts at $5,000 or more. However, many residents qualify for financial assistance through Medicaid or other programs.
Assisted Living Facilities

What They Are
Assisted living facilities are also licensed residential care homes, but they serve larger numbers of residents. They usually support a higher level of care than adult family homes. Many provide 24/7 access to medical care with licensed healthcare professionals on staff to address emergencies, administer medications, and provide physical or occupational therapy.
Beyond caregiving, assisted living facilities sometimes support a rich social life. Many provide regular opportunities to socialize with other residents, enjoy community activities, and do exercise that is appropriate for residents’ abilities.
Who They’re For
Assisted living facilities are a good choice for many individuals who need more intensive medical care than home care or an AFH can provide, but not the 24/7 medical supervision of a nursing home. They can also be a great fit for people who prefer more opportunities to socialize.
Cost
Assisted living facilities also have a highly variable pricing structure. The average monthly cost for an assisted living resident in Washington state was $6,975 in 2024, according to the Genworth Cost of Care Survey. If that’s more than you can afford, you may be able to get financial assistance through Medicaid or other programs.
