Are you a licensed mental health professional (AAC-L, LCSW,LSWAIC, LMHC, LMHC-A) located in Mt. Vernon or Everett?  Sunrise Services is hiring full-time and part-time licensed counselors and social workers for our Geriatric Transitions Program (GTP). GTP is a short-term, 90-day, behavioral health intervention team which supports aging adults with dementia as well as adults with physical disabilities and severe mental illness. Clinicians work both collaboratively with as part of an interdisciplinary team as well as independently with their clients. Schedules are flexible and clinician driven. Maximum caseload capacity is 10. GTP offers challenging but rewarding work, supporting some of the post vulnerable members of our community. We would love to talk to you about joining our team!

Working at Sunrise Services’ Geriatric Transitions Program

As a GTP team member, you work one-on-one with our clients, their support networks, and the people around them to help each client stay in their home and avoid a lengthy hospitalization. Your goal is to ensure each client has the behavioral health support they need to maintain a stable place in the community.

The program aims to establish long-term support and stable placements quickly, so cases typically last 90 days. The GTP team collaborates with each client’s referrer, medical teams, family members, and DSHS.

The Job: Behavioral Health Professional Clinician

Geriatric Transitions

As a GTP Behavioral Health Professional Clinician, you’ll work with up to 10-12 clients at a time and provide intensive support. You can work a flexible schedule based on your own preferences. While you will need to drive to see your clients in the community, you can do your office tasks remotely.

Your responsibilities include:

  • Assessing clients’ presenting problems, needs, risk factors, and barriers to community placement.
  • Providing short-term therapies, counseling, and psychoeducation.
  • Providing mental health education and training to families and caregivers.
  • Establishing a long-term, positive behavioral support plan that helps clients stay in their homes, including care coordination.
  • Crisis stabilization.

Skills and Experience

Our ideal candidate is either a psychiatric nurse or someone with at least a master’s degree in social work, counseling, or one of the social sciences. You should either be a Mental Health Professional (MHP), in the process of working toward MHP status, or someone with at least two years of supervised experience treating people with mental illness or developmental disabilities.

It’s also important to have:

  • Licensing as an Agency Affiliated Counselor.
  • DSHS background clearance.
  • A current valid Washington State driver’s license, insurance, and an acceptable driving record.
  • Microsoft Office skills, including Word and Outlook.
  • Strong written and verbal communication skills.
  • Flexibility.
  • Respect for diversity and commitment to developing multicultural competence and sensitivity.
  • Extensive knowledge of NSHMA protocols.
  • Adherence to professional and ethical standards of practice.

This job pays $32.36-$36.52 per hour, with up to a $3k sign-on bonus.
